1.  Cancellations : There is a  $75 cancellation fee with less than 24 hours notice.
2. Refunds : Refunds are only available if company NDMDT is "at fault". Otherwise, once sample collection is taken, there will be no refund. 
3. Mileage fees : Mileage surcharge starts 30 miles outside of company headquarters., with an initial fee of $15 and a rate of 0.47 cents per mile thereafter.
4. All fees and balances must be paid 4 hours appointment and 2 hours  before collector is dispatched.
